Key points
  • Check applicable tasks against the equipment-specific manual, drawings and approved lists, then schedule daily, weekly, monthly, six-monthly and annual maintenance.
  • For each intervention, record measurements or inspection evidence, actions, the responsible person and closure status. Check marks do not replace inspection records.
  • Obtain consumable and critical-part names, codes, locations and service-life requirements from the bill of materials or approved documents, rather than assumptions.

01Purpose

This form combines scheduled maintenance, work performed, corrective actions and spare-part replacements. Production, equipment and maintenance teams can use it to allocate maintenance time, complete tasks, retain evidence and track outstanding actions and responsibilities.

02Intended users

  • Equipment managers responsible for asset records, periodic plans and shutdown windows.
  • Qualified maintenance personnel performing inspections, lubrication, tightening, adjustments, testing and replacements.
  • Engineering or safety personnel reviewing safety functions, restricted operation, issue closure and conditions for return to service.

03Planning and recording

StepRequirements
1. Establish the equipment baselineRecord the model, serial number, operating environment and maintenance basis. Check applicable tasks against the equipment-specific manual, bill of materials and approved documents.
2. Schedule maintenanceDetermine applicability, planned dates, responsible persons and shutdown windows for each item. Explain exclusions.
3. Isolate safelyUnload the workpiece and lock the equipment. Isolate air or electrical power, release residual pressure and other stored energy, and verify that personnel are appropriately qualified.
4. Perform work and retain evidenceRecord the actual date, measurements, photograph or document references, results, actions, operating restrictions and replacement parts.
5. Close and archiveTrack faults and outstanding actions, complete review and authorized release, update status, and file maintenance records and critical-part histories.

04Maintenance intervals

IntervalTypical tasks
DailyAppearance and work area, energy connections and leaks, tooling, safety functions, unloaded operation and cleaning.
WeeklyFastener and pin retention, hoses and cables, air preparation, tooling accessories and guided movement.
MonthlyLubrication and transmission, braking and locking, tooling safety, travel limits and collision prevention, structural welds, labels and documents.
Every six monthsOverall function under representative loads, pneumatic systems, electrical controls, joints and bearings, and safety protection.
AnnuallyComprehensive inspection and servicing, parameter-to-document consistency, wear-part condition, safety training and annual records.

05Maintenance record requirements

Maintenance and release requirements:This general form does not replace the equipment-specific maintenance manual, drawings, bill of materials, risk assessment or site energy-isolation procedure. Stop using equipment with cracks, deformation, severe wear, abnormal braking or retention, failed safety functions or unexplained noise, and arrange qualified assessment. Spreadsheet formulas indicate completion only and do not establish a safe-to-operate decision.
Recordkeeping principle:Every maintenance activity must be traceable to the equipment, interval, person performing it, evidence, corrective action and final closure status.
